Transition planning and services for young people who were in Wisconsin foster care, starting at age 14 and continuing to age 21 (23 in some cases). Covers education, work, housing, money management, health coverage and obtaining a birth certificate, Social Security card and state ID. Those 18 and older who have left care are served by a regional Transition Resource Agency.

Who it's for

Youth age 14+ in care at least six months; young adults 18 and older who aged out of foster care or were adopted or placed in guardianship after age 16.

How to get in

Youth still in care: contact your county Independent Living Coordinator (map on DCF site). Age 18+: contact the Transition Resource Agency for your region using the DCF TRA map.
