Issues certified Wisconsin birth certificates by mail, online or phone through VitalChek. Mail orders cost $20 for the first copy and $3 for each additional copy of the same record. Walk-in counter service is closed; county Register of Deeds offices also issue many records.
How to get in
Mail the application with a copy of ID to PO Box 309, Madison, WI 53701-0309, or order through VitalChek online or at 877-885-2981 (extra fee).
What to bring
Copy of one: state driver license or ID, U.S. government photo ID, passport, tribal or military ID. Or copies of two: bank or earnings statement, signed lease, health insurance card, utility bill or traffic ticket, vehicle registration or title. Must be unexpired.
