Community Reentry Services, Division of Rehabilitative Programs

Placement is requested through your correctional counselor during annual review.

Extended Foster Care (AB 12)
Lets California foster youth stay in care from 18 to 21 with case management, housing payments, and Independent Living Program services. Youth who already left care can re-enter before 21 by contacting their former social worker or county child welfare agency.

Georgia Statewide Domestic Violence Hotline
A 24-hour line that routes anyone in Georgia dealing with abuse to one of more than 50 local domestic violence programs. Callers can ask about emergency shelter, safety planning, or how to leave; GCADV also runs a program for incarcerated survivors.
La Casa de las Madres
San Francisco domestic violence agency with 24-hour adult and teen crisis lines, a text support line, emergency shelter, drop-in peer counseling, and permanent supportive housing services.
Delancey Street Foundation
Free two-year residential self-help community for people with substance use histories, prior convictions, or homelessness, where residents learn vocational, academic, and interpersonal skills. Applicants write from jail or show up at the building and wait on the bench for an interview.
Father Joe's Villages
San Diego's largest homeless services campus with a daily day center, public lunch, showers, mail, a health clinic, detox and MAT, family shelter intake, and employment help. Show up in person before 11am on a weekday to be scheduled for an assessment.
Center of Hope - Augusta
Emergency shelter in Augusta with separate check-in lines for single women, women with children, men and families. Dinner is served nightly and everyone checks out at 8 a.m.; caseworker appointments can be requested by phone or online.

Extended Foster Care and Aftercare Services
Florida lets young people stay in foster care until 21 and offers Aftercare to anyone who turned 18 in DCF custody, with help available until the 23rd birthday. Aftercare covers mentoring, mental health and substance use support, life-skills and parenting classes, job training and temporary financial aid; a separate tuition exemption runs through age 28 and Medicaid through 26.
Florida's Independent Living Resource Center
Free statewide information-and-referral clearinghouse for current and former foster youth, run by Daniel Kids in Jacksonville. Staff answer questions about Extended Foster Care, PESS, Aftercare, Keys to Independence and other transition benefits and point youth to the right local contact.
Refugee Resettlement and Employment Programs
Reception and placement for newly arrived refugees, community-resource help for Cuban and Haitian entrants, and Match Grant job search, employment coaching and placement for refugees, entrants and asylees. Adult clients are referred through Migration and Refugee Services and the USCCB.

Metropolitan Ministries
Tampa Bay nonprofit serving poor and homeless families and individuals in Hillsborough, Pasco and Pinellas with emergency family shelter, a food market, rent and utility help and job training. Its Metro BrigAIDe mobile outreach team helps with IDs, SNAP and Medicaid applications and housing assessments at more than 20 sites.
Coalition for the Homeless of Central Florida
Low-barrier residential shelter in downtown Orlando serving about 500 men, women and children a day, with free meals, Housing First case management and a diversion program that helps people find housing outside the shelter system.

Jacksonville's largest provider of homeless services, with men's housing downtown on East Adams Street and women's and family housing at Sulzbacher Village in Springfield. Florida-based anti-trafficking organization serving survivors of sex trafficking through street and jail outreach, a sex trade support group, case management, mentorship, a prostitution court diversion program and residential housing with wrap-around services.
Florida Association of Recovery Residences (FARR)
The state's NARR affiliate, which certifies sober living and recovery residences under Florida Statute 397.487. Its online provider list is the place to check whether a recovery residence is certified, and it accepts grievances about residences that fall short of standards.
Harbor House of Central Florida
Certified domestic violence center for the Orlando area with a 24-hour confidential crisis hotline, emergency shelter, help pursuing justice and healing services. The R3 app helps people assess whether a relationship is abusive and where to turn.
United Way 211 Greater Atlanta
Free, confidential referral line staffed around the clock that points Atlanta-area callers to rent and utility help, food pantries, shelter beds, and other services. The team also handles web chat and a texting option for people who cannot talk.
Reentry Services - Operations Support Division
The state unit that plans release for people in Georgia prisons and follows them into the community. Pre-release staff inside GDC facilities identify needs, community coordinators in every judicial circuit link people to housing, jobs and mentors, and the Housing Resources unit runs the THOR and Reentry Partnership Housing beds.
GA RYSE Independent Living Program (Chafee)
DFCS's program for teens and young adults who spent at least six months in Georgia foster care after age 14. An Independent Living Specialist enrolls the youth, builds a transitional living plan, and opens doors to Chafee funds, Education and Training Vouchers, and post-foster-care support up to age 21.
Gateway Center
Downtown Atlanta's main front door for people without housing. It runs coordinated entry for the city, an engagement center with day services, and residential tracks for men including the Upward addiction-recovery program and a two-year veterans program.
The Shepherd's Inn
Shelter and residential program for up to 450 single men a day, with meals, counseling, recovery programming, job training and legal help. Arrive at the Mills Street gate early; pre-screening happens before the weekday morning intake window.
Old Savannah City Mission
Faith-based mission on Bull Street offering three hot meals a day, overnight beds, showers and clothing at no charge, plus a residential recovery program for men coming out of homelessness and addiction. A free medical clinic runs Monday afternoons.
Chatham Savannah Authority for the Homeless
The lead homelessness agency for Savannah and Chatham County. Its street outreach team meets people where they are six days a week and connects them to shelter and housing, and it operates an Access Center for triage at Union Mission's day center on Fahm Street.
The Road Home
Salt Lake County's main emergency shelter provider for single men, single women, and families with children. Beds are assigned through the Homeless Services Line, not by walking up. Operates the Pamela Atkinson Resource Center (South Salt Lake), Gail Miller Resource Center (Salt Lake City), and Connie Crosby Family Resource Center (Midvale).
Garden City Gospel Mission
Men's shelter on Fenwick Street in Augusta that also runs a nine-month residential Life Development Center for men working to leave addiction. Showers are open to the public on Fridays.
Valley Rescue Mission
Columbus nonprofit with a men's transitional shelter, a women and children's shelter, 9–12 month recovery centers for men and women, and a daily hot meal for about 120 people. Shelter and recovery applicants apply online; the men's shelter takes applications by phone.

Parole Reentry Group
IDOC's parole division unit that coordinates housing, community resources, and supportive services for people under supervision after release. Questions about parole conditions or community supervision go to the Parole Office line; general inquiries go to Constituent Services.
DCFS Youth Hotline and Supporting Emancipated Youth
Young adults 18 to 21 whose DCFS case closed can ask to return to care through the Supporting Emancipated Youth program. The same Advocacy Office hotline takes complaints and questions from current and former youth in care.
YCC Family Crisis Center
Ogden's domestic violence and sexual assault agency with a confidential shelter, 24/7 call-or-text crisis line, separate rape crisis line, protective order help, transitional housing, and child care. The public outreach office is on Adams Avenue.
Pacific Garden Mission
Chicago's oldest rescue mission, with separate shelters for men, women, and mothers with children plus addiction recovery and meals. Men's intake runs weekday mornings on a first-come basis; women's beds rarely run out.
The Night Ministry Youth Housing
Overnight emergency shelter (The Crib), interim housing up to 120 days, and long-term programs for Chicagoans ages 18 to 24, including a track for pregnant and parenting youth 16 to 22. Separate numbers for short-term beds and long-term programs.
Safer Foundation
Reentry nonprofit in downtown Chicago offering job training and placement, education, housing support, clothing, and wellness services for people with arrest or conviction records. Enrollment starts through its online client portal.
STOP-IT Program
Anti-trafficking program covering greater Chicagoland, the suburbs, and northern Illinois. Survivors and people who suspect trafficking can call the program hotline directly.
Transition to Adult Living (TAL)
DCFS program for youth leaving Utah foster care. Aftercare support can cover rent, bills, housing deposits, vehicle costs, and crises; education and employment funds, counseling, and medical and substance use supports are also available. Start with your assigned TAL coordinator, or email the coordinator team if you do not know who that is.
