Mail the request with payment to the Springfield office, or contact the county clerk where the birth occurred. Email DPH.VITALS@illinois.gov with questions.
What to bring
Unexpired, readable government photo ID; $10 for a standard certified copy, $15 for the long form, $2 each additional copy
