State office for certified South Carolina birth certificates. The $12 search fee covers one certified copy if the record is found, and every request needs a current photo ID. Mail requests go to the West Columbia P.O. box.

Order online, by mail to Vital Records, P.O. Box 2046, West Columbia, SC 29171, or in person at the Richland County office or a regional DPH office.
What to bring
A valid government, school or employer-issued photo ID and the $12 fee (money order, cashier's check, card, or cash under $50 bills)
