Transitional Services Branch (Chafee / KY RISE)

Kentucky Cabinet for Health and Family Services, DCBS

Supports young people who were in Kentucky out-of-home care at age 14 or older, whether or not they have aged out. Chafee funding pays for help with high school and college, financial literacy, career exploration and job training; formal transition planning starts at 17 and repeats at 19, 20 and 90 days before 21 for youth who stay in care.
