DCYF Services and Supports After Age 18

NH Division for Children, Youth and Families – Adolescent Program

Young adults leaving NH foster, kinship, group, or residential care can stay in care to 21 through the HOPE program, get a LifeSet coach from 17.5 to 23, or enroll in voluntary Aftercare Services with a DCYF worker and Chafee or Education and Training Voucher money for rent, food, insurance, or school.

Who it's for

Ages 18–23 (to 26 if in college or career training) who exited DCYF out-of-home care on or after their 18th birthday, or exited to guardianship or adoption at 16 or older; HOPE requires having been in DHHS custody at 18

How to get in

Contact the nearest DCYF District Office and ask for the Adolescent Worker or Permanency JPPO; HOPE requires signing a Voluntary Placement Agreement
